Services: State Plan Amendment (SPA)

STATE PLAN AMMENDMENT (SPA):

Medical assistance waiver program that expands eligibility for free family planning services & supplies to women, men and adolescents. Eligibility requirements include: US citizens, PA residents, currently not pregnant with limited or no health insurance and meet income guidelines at 214% of poverty.

 

Services Available

  • Birth control
  • Pap tests
  • Emergency contraception (“Morning After Pill”)
  • STI testing & treatment
  • HIV testing & counseling
  • Breast exams
  • Cervical and testicular cancer prevention and screening

How to get started

An application process must be completed to enroll in Medicaid and Family Planning Services under DHS. Lebanon Family Health Services is happy to enroll eligible individuals into this program. To accurately complete the application at our office, please bring the following items (if unable to locate, please bring the information that is readily available to you):

  • Social Security Number
  • PA Driver’s License or state-issued identification card
  • Birth certificate or U.S. passport
  • A recent pay stub or employer’s letter stating pay for a typical pay period for all working family members in your household*
  • Proof of other income, such as child support
  • Insurance card (if you have health insurance)

*Household size includes the individual’s legal spouse and/or dependent children
