Lebanon Family Health Services offers an array of services and programs related to reproductive health care. Visits are private and confidential. Our services are provided by female staff, including certified registered nurse practitioners, RNs (registered nurses), LPNs (Licensed Practical Nurses) and Clinician Assistants.

For Your Information

Fees for our services are based on a sliding fee scale, determined by household size and income. Services may be free or at a reduced fee, but even at our full fee, the cost for our services is about half that of private physicians and pharmacies. No one is refused services for the inability to pay based on Federal poverty guidelines. We also accept a variety of private insurance plans and State Medicaid Plans including:

STI Testing

Sexually transmitted infection (STI) testing is an important part of preventive health! It is important to get tested, even though you do not have symptoms! Lebanon Family Health Services offers testing with no out of pocket costs.

IMPORTANT ANNOUNCEMENT PLEASE READ

There will be significant changes to our HIV/STI testing program going into effect October 1, 2022. Due to a change in the contract and expectations from the Department of Health, our free testing will only be available to those who are uninsured, those who express financial hardship and unable to afford their co-pay or other insurance expenses, and those who share their insurance and do not wish to show their services on the insurance explanation of benefits statement. Please read below for more specific information.

Uninsured Patients    

  • Appointments required Monday – Thursday, 8:00-4:00. Walk ins accepted on Friday 8:00-3:00

  • If you have had contact with someone with a known STI
  • If your symptoms continue after receiving treatment for an STI

Insured

  4. If your insurance does not cover testing or there are limitations with your coverage, please talk with our staff to discuss your options

We understand this is a drastic change to how Lebanon Family Health Services has always provided testing services. Your health is of utmost importance to us, and we will do everything we can to ensure our services are available to anyone that requires testing.
